Basin Kings Grappling Liability Waiver, Assumption of Risk, and Media Release

1. Assumption of Risk

I understand that Brazilian Jiu-Jitsu (BJJ) is a full-contact grappling art that involves significant physical exertion and close bodily contact. I acknowledge that there are inherent risks associated with participating in BJJ classes, sparring (rolling), and conditioning. These risks include, but are not limited to:

• Physical Injury: Scratches, bruises, sprains, strains, joint dislocations, broken bones, concussions, and in rare cases, serious injury or death.

• Skin Infections: Ringworm, staph (MRSA), impetigo, herpes gladiatorum, and other communicable skin diseases common in grappling sports.

• Viruses: Influenza, COVID-19, and other respiratory illnesses due to close proximity.

I voluntarily choose to participate in these activities with full knowledge of the risks involved.

2. Release of Liability

In consideration of being allowed to participate in activities at Basin Kings Grappling, I hereby release, waive, discharge, and covenant not to sue Basin Kings Grappling, its owners, instructors, employees, and volunteers (collectively the "Releasees") from any and all liability, claims, demands, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, whether caused by the negligence of the Releasees or otherwise, while participating in such activity or while in, on, or upon the premises.

3. Medical Representation

I certify that I am physically fit and have no medical condition that would prevent my full participation in BJJ training. I agree to inform the instructor of any pre-existing medical conditions or injuries prior to training. I understand that it is my responsibility to discontinue training if I experience pain or discomfort.

4. Hygiene and infectious Disease Policy

I agree that I will not attend training if I have any open wounds, active skin infections, or symptoms of illness (fever, cough, etc.). I understand that Basin Kings Grappling reserves the right to remove any student from the mats who presents a hygiene risk to other students.

5. Media Release (Photo & Video)

I hereby grant Basin Kings Grappling and its representatives the irrevocable right and permission to use photographs, video recordings, and/or audio recordings of me on gym premises or at gym events.

• Usage: These images/videos may be used for educational, promotional, marketing, and commercial purposes, including but not limited to: social media (Instagram, Facebook, YouTube, TikTok), the gym website, flyers, and digital advertisements.

• Compensation: I understand that I will not receive any compensation, royalties, or financial payment for the use of these materials.

• Waiver: I waive the right to inspect or approve the finished product wherein my likeness appears.

6. Code of Conduct

I agree to abide by the rules and etiquette of Basin Kings Grappling. I understand that the practice of BJJ requires trust between training partners. I agree to respect my training partners, apply submissions with control, and release holds immediately when my partner "taps" (signals submission).

Acknowledgement of Understanding

I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue.

FOR PARTICIPANTS UNDER 18 (PARENT/GUARDIAN CONSENT)

I am the parent or legal guardian of the minor named above. I have read this agreement and fully understand its terms. I hereby give my permission for my child to participate and agree to bind myself and the minor to all terms of this agreement, including the Media Release and Release of Liability.
